HCL Mental Health are looking for a band 6 CPN to work in Rotherham. This role is based in the Memory Service. Experience with older dementia patients is an advantage. The role is a mixture of clinics and visiting peoples homes. Any experience should be post registration. You will complete initial assessments, cognitive testing and monitoring of the needs of clients experiencing memory problems. Deliver diagnosis and provide post diagnosis support. Provide ongoing education/guidance/support to clients and carers both during and after assessment.
